29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards:  a) Second floor, IDVE area: Employees were exposed to a 15 foot fall hazard when the guardrail gate was open for receiving materials, as observed on or about January 16, 2015.  b) Second floor, IDVE area: No toe boards were installed along the side guardrails of the receiving floor opening to prevent materials from falling and striking employees working below, as observed on or about January 16, 2015.  Abatement Certification Required.
