OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to struck by hazards:     (a) Red Assembly, Hydraulic Press #1:  Employees were exposed to being struck by flying parts in the machine area for Hydraulic Press #1 Nugier 40 ton hydraulic press.  On or about January 19, 2015, a metal cylindrical spacer was used to extend the ram.     (b) Red Assembly, Hydraulic Press #2:  Employees were exposed to being struck by flying parts in the machine area for Hydraulic Press #2.  Hydraulic Press #2 was also being used with a metal cylindrical spacer to extend the ram.      (c) Blue Assembly, Hydraulic Press #3:  Employees were exposed to being struck by flying parts in the machine area for Hydraulic Press #3.

(a)  3715 E Farm Road 94, Springfield, Missouri:  On or about January 20, 2015, the employer failed to provide a copy of the first report of injury to the OSHA Compliance Safety and Health Officer for an accident that occurred on January 19, 2015.  Copies of the first report of injury was not provided to the OSHA Compliance Officer to take with the officer within four business hours of the request.
