29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:   a. On or about January 21, 2015, employees were exposed to the hazard of falling approximately 7-1/2 feet while using the rails and cross bracing of a walk-thru arch type scaffold to gain access to the working levels of the scaffold.

On or about January 21, 2015, an employee was exposed to the hazard of falling approximately 22 feet while working on the top level of a 3-tierd scaffold without being protected from falling.

On or about January 21, 2015, an employee was exposed to the hazard of falling approximately 7-1/2 feet while using a portable extension ladder, which did not extend 3 feet past the landing surface, to gain access to the working level of the walk-thru arch type scaffold.
