29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    a- Building A - Modular Home Section 27501-A, 301 Riverside Drive, Millville NJ: On or about 01/21/15 a company employee (without any fall protection) performing shrink wrap installation and other related activities while standing on top of a modular home section, was exposed to fall hazards in excess of 142 inches to the ground surface below.      b- Building D - Station 12, 301 Riverside Drive, Millville NJ:  On or about 02/11/15 a company employee (without any fall protection) performing HVAC related installation and other related activities while standing on top of a modular home section, was exposed to fall hazards in excess of 72 inches to the ground surface below.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
