29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a) At the facility, on or about February 26, 2015 -  Fork lifts with modifications that affected the capacity and safe operations of the fork lifts did not have the manufacturer's approval prior to use.  Employees use boom attachments on fork lifts to lift metal wire.
