29 CFR 1910.213(i)(1): All portion of the saw blade were not enclosed or guarded, except for the working portion of the blade between the bottom of the guide rolls and the table: a) At 1106 Guinotte in Kansas City, MO 64120 (Machine Room) - An employee was using a "Powermatic" band saw (Model No: 141; S/N: 3-1078) where 8 and one-half inches of the blade was unguarded, including the blade guide wheels, and the employee was cutting 1/4 inch plastic, exposing the employee to cut and/or amputation injuries from the unused portion of the blade.

29 CFR 1910.215(a)(4): Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage: a) At 1106 Guinotte in Kansas City, MO 64120 (Machine Room) - An employee was using a "Wissota" ball bearing bench grinder (Model: E-7; S/N: W11-76; 3450 R.P.M.) for grinding machine parts, and the work rest was not adjusted to 1/8 inch or less, exposing the employee to flying object hazards in the event the abrasive wheel were to shatter.
