29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:   Power Shack:  On or about 23 January 2015 and times prior thereto, the 120 electrical panel, identification number LVDP was not properly identified with a listing for the circuits.  This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision:  Power shack:  on or about 23 January 2015 and times prior thereto, live wires were exposed from flexible conduit routed down the wall of the shack.  This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1917.11(d): Dunnage, lumber, or shoring material had protruding nails that were harmful to the employees.    Samson dock, receiving hold area:  on or about 23 January 2015 and times prior thereto, employer did not ensure that dunnage, lumber, and shoring material had protruding nails removed from the immediate work area. In the shop bay area, a wooden pallet had nails approximately two inches in length, protruding upward. This condition exposed employees to a puncture hazard.
