29 CFR 1910.106(e)(6)(i): Adequate precautions  were not taken to prevent the ignition of flammable vapors. Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat.  (a)Foster Trucking, Inc. @ Wells Ranch, #33-14- 225-228, Tank Battery, Gill, CO 80624:  On and before January 2nd, 2015, the employer did not ensure adequate precautions were taken to prevent the ignition of flammable vapors while conducting a tie-in and blow down activities as a result of having to work on a frozen gas line on a tank battery production site. High pressure hydrocarbon liquids and hydrocarbon vapors were present during work activities. This condition resulted in a fire and caused a vacuum truck operator to sustain 2nd and 3rd degree burns.

29 CFR 1910.1200(h)(1): Employer had not provided employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area:    (a) Foster Trucking, Inc. @ Well Ranch, #33-14- 225-228, Tank Battery, Gill, CO 80624:  On and before January 2nd, 2015, the employer did not ensure that each employee had been provided with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area.  On January 2nd, 2015, a vacuum water truck operator conducted tie-ins and blow downs activities at the tank battery production site. The employee performed these tasks with the 24 inch man way hatch open and within close proximity to truck blow down vents that were open and high pressure hydrocarbon liquids and hydrocarbon vapors were present. The employer did not train each employee with regards to oxygen deficient atmospheres, which are Immediately Dangerous to Life or Health (IDLH).  This condition exposed employees to a respiratory hazard.    Abatement Note:  The employer shall provide and ensure that employees use four gas meters when engaged in tie-ins, blow downs, or any operation that may expose them to hydrocarbon gases or vapors or oxygen deficient atmospheres.
