29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a.  On or about January 9, 2015 at a new residential jobsite, a Lead working near an elevator shaft was not protected from a 16 feet fall hazard by the use of a personal fall protection system.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   a.  On or about January 14, 2015 at Northwest Florida AC, LLC, the employer failed to inform OSHA within 24 hours after an inpatient hospitalization of one of their employees from a work related accident. The employer was advised of the hospitalization January 9, 2015 and OSHA was notified by the co-owner on January 14, 2015.
