29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  Jobsite located at 12202 S 144th Street, Omaha, NE 68138; In the server room: The employer failed to ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug were inspected before each day's use. On or about January 28, 2015 a flexible cord set being used by an employee to energize a Shop-Vac was found to have an open ground and discovered that the flexible cords in the set were all missing ground pins.
