29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: a) 30 Pine St., Gardner, MA: On or about January 30, 2015, a scaffold plank broke while an employee was working from that plank, resulting in his falling to the ground, more than 50 feet below. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.451(b)(5)(ii): A platform greater than 10 feet in length extended over its support more than 18 inches (46 cm), without being designed and/or installed so that the cantilevered portion of the platform was able to support employees without tipping, nor were their guardrails which blocked employee access to the cantilevered end: a) 30 Pine St., Gardner, MA: On or about January 30, 2015, a 20 foot long wood scaffold plank was cantilevered more than 30 inches over its support and was not able to support employees without tipping, nor were there guardrails which blocked employee access to the cantilevered end(s). Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
