29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    WORKSHOP:  The bar feed entrance of the Mori Seiki, NLX2500SY, horizontal CNC lathe was not appropriately protected and/or guarded to protect the operator and/or other employees from the hazard created by rotating part/bar.      On/or about January 15, 2015, the forearm of the operator of the above machine was hit by the rotating bar while manually operating the machine, and sustained severe injuries for the incident.    ABATED DURING THE INSPECTION.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.    ESTABLISHMENT:  The employer had not reported to OSHA within 24 hours of the incident that happened on January 15, 2015 whereby a forearm of the operator of the Mori Seiki horizontal lathe, NLX2500SY, was hit by a rotating bar, sustained sever injuries and resulted in-patient hospitalization.     ABATED DURING THE INSPECTION.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met.    WORKSHOP: The employer had not established and implemented a written hazard communication program where employees were required to work with chemicals, such as (but not limited to) the oils.
