29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2).   On the Ex: USS Shenandoah, at the second deck port side 2-111-3 employees conducted dismantling operations next to an inadequately guarded opening without a mid-rail in place, the fall distance into harbor water was greater than fifteen feet in height.
