29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) 3 Greenwich Drive, Jersey City   Employees working on the 4th working level dismantling a tubular welded framed scaffold were not wearing hard hats. Other employees were working on the level directly above them removing planking units and passing them down.   Violation observed on or about 01/23/2015

29 CFR 1926.451(e)(9)(iv): Cross braces on tubular welded frame scaffolds were used as a means of access or egress.  a) 3 Greenwich Drive, Jersey City   Employees dismantling a tubular welded framed scaffold approximately 36ft in height using the cross braces as a means of egress, were exposed to fall hazards.   Violation observed on or about 01/23/2015

29 CFR 1926.451(g)(2): The employer did not provide fall protection for employees erecting or dismantling supported scaffolds:  a) 3 Greenwich Drive, Jersey City   Employees were dismantling a tubular welded framed scaffold on the 4th level. The cross bracing units were being used as the top rail of the guardrail system however there was no mid rail present, thereby exposing employees to an approximate 24ft fall hazard.  Violation observed on or about 01/23/2015
