29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. Employees at elevations of six feet or more above the ground are not protected from fall hazards by a fall protection system such as but not limited to personal fall arrest system, safety net system or guardrail system.  The employer fails to assure effective protection against falls as required by the standard, as demonstrated on February 5, 2015, when employees were installing masonry grout to concrete roofing tiles on a residential roof with a 7:12 pitch at Lot 154, 13007 Trave Way, Jacksonville, Florida 32246 without fall protection and were exposed to fall hazards of 10-foot, 6 inches and 17 foot, 6 inches above the lower level.  Professional Sunshine Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1002905, citation number 01, item number 001 and was affirmed as a final order on November 20, 2014, with respect to a workplace located at 3632 Valverde Circle, Jacksonville, FL.  Professional Sunshine Roofing, Inc. was previously cited for a repeated violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 907966, citation number 02, item number 001 and was affirmed as a final order on July 10, 2013, with respect to a workplace located at 1398 Sunset View Lane, Jacksonville, FL.  Professional Sunshine Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 799521, citation number 01, item number 001 and was affirmed as a final order on January 28, 2013, with respect to a workplace located at 13507 Montecito Place, Jacksonville, FL.
