On or about 02/04/2015, at the site assembly area - one relocatable power tap was connected in series to another relocatable power tap.      b. On or about 02/04/2015, at the site assembly area - relocatable power taps were connected to extension cords and not directly to a branch circuit receptacle.      c. On or about 02/04/2015, at the site assembly area - relocatable power taps were mounted or permanently attached using screws to the metal columns holding the tipouts in place.
