29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by points of operation: a) Main Floor, OTC 1 Department: Preco Machine Number 1 and 2. The guard over the three feed rollers and the stamping section was not fully extended exposing the employees to the hazards associated with the point of operation, on or about 2/6/2015. b) OTC # 2 Department: The small "Jones" number 1 machine: The safety interlock on the access gate of the machine door was malfunctioning allowing the employee to enter thru the door while the machine was running; on or about 2/6/15.
