29 CFR 1910.1200(h)(3): Employee training did not include the requirements of 29 CFR 1910.1200(h)(3)(i) through (h)(3)(iv): a) In the Gas Phase Unit, employee training did not include the methods and observations that may be used to detect the presence or release of carbon monoxide from the gas phase "A" kill system; the simple asphyxiation hazards of carbon monoxide; and the measures employees could take to protect themselves from the hazards of carbon monoxide, including specific procedures the employer has implemented to protect employees from exposure to carbon monoxide, such as appropriate work practices, emergency procedures, and personal protective equipment to be used.
