On the Kitchen side of the doorway, the layout of equipment and the location of the doorway made it difficult for employees to maneuver through the doorway with pallet jacks loaded with tanks, exposing employees to the hazard of getting caught between the pallet jack and a stationary object, such as a wall or piece of equipment, on or about February 9, 2015.  No abatement certification or documentation required.
