Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the lifting device fails:    a)  Single Stream Sorting Area, on or about 2/11/15:  A 1 ton and a 1/4 ton Dayton overhead hoists was being used to lift the Shafting System from the Newspaper Sorting System to replace the rubber stars and with hoist have not been inspected on a frequent and periodic basis.    Among other methods one feasible and acceptable method to correct this hazard is to:    1. Conduct frequent and periodic inspections of the hoists for wear, deteriorations and malfunction as per ASME B30.16-2012, Section 16-2.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created point of operation, rotating parts, and flying chips:      a)  Weld Shop, on or about 2/11/15:  Dayton Band Saw was not protected with a guard on the unused portion of the blade on the left side.

29 CFR 1910.304(g)(5):  The path to ground from circuits, equipment, and enclosures was not permanent, continuous and effective:     a)  Single Stream Sorting Area, on or about 2/11/15:  Two Metal Stand Floor Fans had ground pins missing from plug ends.       b) Container Sorting Area, on or about 2/11/15:  Two Metal Stand Floor Fans had ground pins missing from plug ends.
