29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: On February 3, 2015 and at times prior thereto; front of residential home (west side); employees were exposed to a potential fall of 17 foot to ground level while working on an unguarded ladder-jack scaffold system while installing the shingles and/or flashing.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On February 3, 2015 and at times prior thereto; front of house west area; employees conducting roofing work on a residential home were observed working on a 10:12 roof without fall protection to prevent them from falling 25 feet to ground level.
