Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire or explosion hazards from dust collectors that were located inside the building and were positioned near occupied work areas and the means of egress:    A.)  Erie Wood Products, LLC, 1835 East 12th Street, Erie, PA  16511, east side of the production area, on or about February 12, 2015 and times prior thereto:  Employees who perform work activities, including, but not limited to, woodworking were exposed to a potential fire or explosion hazards from wood dust in a Dustek enclosureless dust collection system.  The Dustek enclosureless dust collection system is 9.5-feet from the exit door and the pathway (i.e. exit access) that led to the exit door.

The Brewer Gang Saw's procedure does not include specific instructions for locking out pneumatics, verifying that pneumatics have been safely locked out, and instructions for returning pneumatic energy to the machine following service and maintenance.
