29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about February 12, 2015, the employer failed to ensure that effective guarding was in place over the portion of the blade in which cutting action was not taking place on the JET horizontal bandsaw.     b) On or about February 12, 2015, the employer failed to ensure that effective guarding was in place over the pulley drive system of the blade on the JET horizontal bandsaw. The access door was not permanently affixed with a locking mechanism. Operators were able to open the door without the use of a special tool.

Polystar Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(g)(2), which was contained in OSHA inspection number 111323, citation number 1, item number 6 and was affirmed as a final order on May 17, 2015, with respect to a workplace located at 2030 Midway Dr.  Twinsburg, OH 44087.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    On or about February 12, 2015, the employer did not provide hazard communication training to employees exposed to hazardous chemicals such as, but not limited to, paint containing styrene, primers containing isocyanates, pigments, welding gases, anti-spatter, and liquefied petroleum gas.        Polystar Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(h)(1), which was contained in OSHA inspection number 111323, citation number 1, item number 9-d and was affirmed as a final order on May 17, 2015, with respect to a workplace located at 2030 Midway Dr.  Twinsburg, OH 44087.
