29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a) On or about February 5, 2015 machine guarding on the Sheidan press was inadequate in that the guards opening allowed for the operators finger(s) to enter the pinch points.    b)  Guarding on the Lone Packer press was inadequate in that the guards opening allowed for the operators finger(s) to enter the pinch points.

29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.    a) Guarding was not in place over the main motor drive shaft on the Cameron rewinder machine exposing employees to rotating parts.    b) Guarding was not in place over the floor mounted motor drive rotating shaft on the Cameron rewinder machine exposing employees to rotating parts.     c) Guarding was not in place over the extended shafts coming out of the main motor drive on Packer #2, front and rear  sections.

Where the drive extends over other machine or working areas, protection against falling shall be provided. 29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means:      a) The cord attached to the electrical junction box for the Cameron rewinder was not guarded from contact with the exposed wires, in that the wires was not covered and enclosed inside of the box.     b) The main motor drive for the Langston rewinder was not guarded from contact with the exposed wires, in that the wires was not covered and enclosed inside of the box.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.      a) Flexible cord attached to the main motor drive on the Cameron rewinder was not equipped with a strain relief device to prevent movement and/or damage to conductors.    b) Flexible cord attached to the main motor drive on the Labor Saber was not equipped with a strain relief device to prevent movement and/or damage to conductors.    c) Flexible cord attached to a junction box attached to the column between the press #2, was not equipped with a strain relief device to prevent movement and/or damage to conductors.
