29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Jobsite, 8601 Henry Ave., Philadelphia - The portable ladder used to access the roof of the building adjoining the Applebee's did not extend at least three (3) feet above the roof landing, exposing employees to a fall hazard of approximately fifteen (15) feet during access and/or egress. Employees were installing a roof cap on the Applebee's restaurant. Observed on or about 2/16/15. b) Jobsite, 8601 Henry Ave., Philadelphia - The A-frame ladders used to access the steep roof from the flat roofs on both sides of the steep roof did not extend at least three (3) feet above the roof landing, exposing employees to fall hazards up to approximately six (6) feet during access and/or egress. Employees were installing a roof cap on the steep roof. Observed on or about 2/16/15.
