29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) Jobsite, 8601 Henry Ave., Philadelphia - Employees installing a roof cap on a steep roof were not protected by guardrails, safety net systems or personal fall arrest systems, exposing employees to fall hazards up to approximately twenty (20) feet.  Observed on or about 2/16/15.

Observed on or about 2/16/15.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Jobsite, 8601 Henry Ave., Philadelphia -  The portable ladder used to access the roof of the building adjoining the Applebee's did not extend at least three (3) feet above the roof landing, exposing employees to a fall hazard of approximately fifteen (15) feet during access and/or egress.   Employees were installing a roof cap on the Applebee's restaurant.  Observed on or about 2/16/15.    b) Jobsite, 8601 Henry Ave., Philadelphia - The A-frame ladders used to access the steep roof from the flat roofs on both sides of the steep roof did not extend at least three (3) feet above the roof landing, exposing employees to fall hazards up to approximately six (6) feet during access and/or egress.  Employees were installing a roof cap on the steep roof.  Observed on or about 2/16/15.
