29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about February 19,2015, employees were exposed to struck-by hazards from items such as but not limited to: pneumatic screw guns and sharp sheet metal, when not wearing protective helmets, while other employees were working above them on the scaffold work platform installing sheet metal siding to the building.

29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design:   On or about February 19, 2015, employees were exposed to falls greater than 10 feet in height to the ground level below, when working from a personnel work platform (scaffold) on the tines of a Sky Trak 6036 all-terrain forklift #11714, which was 122 inches wider than the all-terrain forklift's width, while installing sheet metal siding.

29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:   On or about February 19, 2015, employees were exposed to falls greater than 10 feet in height to the ground level below, when working from a work platform (scaffold) which was not attached to the forks of a Sky Trak 6036 all-terrain forklift #11714 and the forklift was moving horizontally when bringing employees closer to the building, while installing sheet metal siding.

29 CFR 1926.351(d)(1): Electrodes (rods, etc.) were not removed from the electrode holder when the holder was left unattended:  On or about February 19, 2015, employees were exposed to electrical shock  hazards of 120-240 volts, when the electrode was left on the electrode holder unattended, while employee was using a cutting torch in the vicinity of the electrode holder.
