OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  F/V Bristol Leader, Coastal dock; on or about 9 February 2015 and times prior thereto; five instances were noted where electrical hazards existed:       (a) Engineering room/area:  The 208/120 electrical panels L1 and L2 were missing covers.          (b) Case up area:  Sewing machine electrical cover partially connected, exposing energized electrical wires that were not guarded.         (c) lazarrette:  Grey extension cord did not have a ground connector and had evidence of damaged insulation.       (d) Thruster room;  480 volt, 3-phase Emergency Generator panel missing cover.    This condition exposed employees to and electrical shock and burn hazard.
