OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  F/v Frontier Mariner, OSI dock, Engineering area;  On or about 10 February 2015 and times prior thereto, the 208/120 volt Reefer Compressor #2 electrical box was missing a dead front.  This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1918.35: Open weather deck hatches around which employees must work that are not protected to a height of 24 inches (.61 m) by coamings shall be guarded by taut lines or barricades at a height of 36 to 42 inches (.91 to 1.07 m) above the deck, except on the side on which cargo is being worked. Any portable stanchions or uprights used shall be supported or secured to prevent accidental dislodgement.  [62 FR 40101, July 25, 1997]   (a)  F/V Frontier Mariner, OSI Dock, main deck area:  On or about 10 February 2015 and times prior thereto, the rope railing was not properly tightened.  This condition exposed employees to a 10 foot fall hazard.  (b)  F/V Frontier Mariner, OSI Dock, wheel house deck area:  On or about 10 February 2015 and times prior thereto, the 10 foot access ladder was not guarded with a railing.  This condition exposed employees to a 10 foot fall hazard.
