29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level:  Gear locker:  On or about February 7, 2015 and times prior thereto, the heater fan was not properly guarded and there were gloves and other gear right next to heater.  This exposed employees to laceration/amputation hazards to their hands.

29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION/CONDITION) (DESCRIBE HAZARD)    Vessel:  On or about February 7, 2015 and times prior thereto, employees had not been fit tested for respirator use.  This exposed them to chemical hazards in the work place if respirator did not fit properly.
