29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: Factory: On or about February 9, 2015 and times prior thereto, employees were using super red and harbor master without an eyewash in the area. This exposed employees to eye injuries from not having suitable flushing capabilities for immediate use.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.I. Maintenance Shop: On or about February 9, 2015 and times prior thereto, an air nozzle used for cleaning was operating at 90 p.s.i. This exposed employees to laceration/air embolism hazard from using an air nozzle that would not reduce if dead-ended with skin.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: Vessel; On or about February 9, 2015 and times prior thereto, the employer had not implemented a written respiratory program for employees who wore tight fitting respirator. Employees had not been fit tested or had a medical evaluation and were not storing them properly. This exposed employees to respiratory hazards from improper use.
