29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :    Reefer Flat:  On or about February 9, 2015 and times prior thereto, employees were exposed to over 97 decibels on a daily basis and were not receiving annual hearing tests.  This exposed employees to occupational noise exposure hazard without follow-up evaluations to test for hearing loss.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Factory:  On or about February 10, 2015 and times prior thereto, employees were using super red and harbor master without a properly operating eyewash in the area.  This exposed employees to eye injuries from not having suitable flushing capabilities for immediate use.

29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v):  (a)  Factory Area:  On or about 2/4/2015, and times prior thereto, an unmarked container of harbormaster was used.  This exposed employees to chemical hazards from an unknown source.
