29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a) Robinson Pipe Services, INC. working at the Ford Plant located at 640 Miller Rd Avon Lake Ohio 44012; on or about February 21, 2015, the employer did not ensure that the employees were provided and were using the types of PPE that would protect the affected employees from the hazards of being struck by the approximately 40,000 PSI water discharge from operating the JETSTREAM high pressure washer.  An employee suffered a struck by injury on his foot from the discharge of the water blasting operation during paint removal from metal surfaces in a paint booth. The boots that all employees were wearing were not designed and manufactured to withstand the operating pressures being used.

29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employees from the hazards identified in the hazard assessment:    (a) Robinson Pipe Services, INC., working at the Ford Plant located at 640 Miller Rd Avon Lake Ohio 44012; on or about February 21, 2015, the employer did not conduct a proper hazard assessment to identify the proper personal protective equipment that would protect the employees from the hazards of being struck by the approximately 40,000 PSI water discharge from operating the JETSTREAM high pressure washer.  An employee suffered a struck by injury on their foot from the discharge of the water blasting operation during paint removal from metal surfaces in a paint booth. The boots that all employees were wearing were not designed to withstand the operating pressures being used.     (b) Robinson Pipe Services, INC., on or about February 26, 2015, the employer's hazard assessment did not assess the hazards posed to the employee's trunk, arms, legs, and head to assure the appropriate protective equipment was selected and used while operating a 40,000 PSI JETSTREAM high pressure washer. Employees were exposed to struck by injuries from high pressured water when conducting this task.
