(b) Large cutter section, east side on the production floor:  On or about February 26, 2015, and at times prior thereto, an employee was exposed to a caught in hazard, while using a Southworth lift table to lift paper pallets that did not have a guard to protect the employee from getting his foot caught between the table and the floor.
