29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:    (a) Round cornering work section, southwest corner of the production floor:  On or about February 26, 2015, and at times prior thereto, an employee was exposed to a caught in hazard, while using the Lawson Super Duty Drill that had an exposed rotating shaft, within 1 foot of where the employee stood during drill operation.    (b) Large cutter section, east side on the production floor:  On or about February 26, 2015, and at times prior thereto, an employee was exposed to a caught in hazard, while using a Southworth lift table to lift paper pallets that did not have a guard to protect the employee from getting his foot caught between the table and the floor.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and cables were run through holes in walls, ceilings, or floors:  Press area, southeast corner of the building:  On or about February 26, 2015, and at times prior thereto, employees were exposed to an electrical hazard, while checking the level of the outside press waste water dumping station, that had a flexible 120 volt cord that was routed through a hole in the wall.
