29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  On or about March 5, 2015, employees were exposed to a fall hazard in excess of 4 feet when carrying a 5-6ft long metal pipe up a ladder on the side of rolling stock tanker trailers due to lack of a fall harness with a retractable lifeline.  Abatement certification required per 29 CFR 1903.19(c).
