The four inch receptacle box with knockouts is not listed and labeled to be used at the end of a flexible cord.

29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: a) On or about 03-04-15 at 1324 S. 26th Street in Manitowoc, employees were walking/working from surfaces approximately five (5) inches wide at approximately seven (7) feet above concrete surface without utilizing fall protection equipment. Employees were walking on the top surface of tank rims to reach and pull equipment off the forks of a forklift.

29 CFR 1926.502(d)(15)(i): Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two: a) On or about 03-06-15 at 1324 S. 26th Street in Manitowoc, an employee was utilizing a damaged synthetic rigging sling as the anchor point of a personal fall arrest system at approximately fifteen (15) feet above surface level. The sling was not designed as a part of a fall arrest system.
