29 CFR 1926.960(c)(1)(iii)(B): The energized part was not insulated from the employee and from any other conductive object at a different potential:  a) On or about 03/10/2015, at the job site, (Wood pole marker #65) an employee working from the basket of a truck aerial lift, installing a new cross arm on a concrete pole and moving the existing power distribution lines from a wood pole to the concrete pole was electrocuted when the energized lines were not properly insulated and made contact with the employee.
