29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) 11 Maple Street, Helmetta, NJ 08828. Employees were allowed to work below sheet steel roofing panels while they were being installed overhead with no head protection in place, i.e. Hard Hat exposing the employee to head injuries from falling materials.       Condition occurred on or about 03/24/15.

29 CFR 1926.451(c)(2)(v) Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied:    a) 11 Maple Street, Helmetta, NJ 08828. Employees were allowed to work on and from a job-made scaffold platform raised by the forks of a Bobcat T300 Skid Steer, which was not secured to prevent cantilevering exposing the employee to fall hazards.       Condition occurred on or about 03/24/15.
