29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water.:  a) Dock - On or about 4/2/15, employees were exposed to fall hazards from an opening adjacent to the walkway.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).   The LaFarge Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1917.112(b)(1), which was contained in OSHA inspection number 301417622, citation number 1, item number 1a and was affirmed as a final order on 5/21/10, with respect to a workplace located at 1215 E 2nd Street, Owensboro, KY 42303.

In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
