On March 30, 2015 an employee was fatally injured when he was struck-by a falling pile.

29 CFR 1926.603(c)(5): When it was necessary to cut off the tops of driven piles, pile driving operations were not suspended except where the cutting operations were located at least twice the length of the longest pile from the driver: a) 100 Green Street, Marcus Hook, PA - Tank 40-TK-102 - The employer did not suspend pile driving operations when cutting operations were located within twice the length of the longest pile from the pile driver. On March 30, 2015, a contractor's employee was fatally injured when he was struck by a falling pile during a cutting operation.
