29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard in the workplace:  (a)Liberty Oilfield Services LLC, at Well Site SRC Kiehn #22-4CHZ, Weld County Roads 7 and 48, Berthoud, CO: On and preceding 4/6/15, the employer did not identify and evaluate the respiratory hazard in the workplace.  Employees engaged in hydraulic fracturing of oil and gas wells used crystalline silica (87-99.9%) as part of the process.  The transport and blending of the hydraulic fracturing materials released respirable silica and respirable dust.  Employee exposure to respirable silica or respirable dust had not been determined.  This condition potentially exposed employees to a respiratory hazard.
