29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance. a. Located in the facility at 804 Byesville Road in Cambridge Ohio, the employer did not ensure that the Mixer 1 Lock Out procedures were adequate to include (but not limited to) a specific statement of the intended use of the procedure; specific procedural steps for shutting down, isolating and controlling hazardous energy; specific procedural steps for the placement, removal and transfer of lockout devices; specific requirements for testing and verifying the effectiveness of the lockout devices another energy control measures for maintenance and contractors performing repairs and service.
