This exposed employees to a potential shock hazard.

29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:  (a) Brown Warehouse:  On or about April 8, 2015 the inspection revealed that employees were potentially exposed to serious injury from a fall to a lower level because the District Ranger did not ensure that a midrail was in place on the railing system. Located on the second story mezzanine above the storage area.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 978142, notice number 1, item number 1 and was affirmed as a final order on 12/30/2014 with respect to a workplace located at Bend Pine Administration Site in Bend, Oregon.

This exposed employees to potential shock/burn hazards if employee contacted the electrical connections.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 978170, notice number 1, item number 2 and was affirmed as a final order on 12/23/2014, with respect to a workplace located at Redmond Air Center Redmond, Oregon.  The Forest Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 903968, notice number 1, item number 2 and was affirmed as a final order on 07/01/2013, with respect to a workplace located at Pine Orchards in Cottage Grove, Oregon.

(c)  Brown Warehouse gym far wall:  On or about April 8, 2015 and times prior thereto, the District Ranger did not ensure that an electrical junction box had a cover over an opening. This exposed employees to potential shock/burn hazards if an employee were to contact the electrical connections.  (d)  Maintenance building front heater room:  On or about April 8, 2015 and times prior thereto, the District Ranger did not ensure that an electrical junction box had a cover over an opening. This exposed employees to potential shock/burn hazards if an employee were to contact the electrical connections.  (e)  Maintenance building back heater room:  On or about April 8, 2015 and times prior thereto, the District Ranger did not ensure that an electrical junction box had a cover over an opening. This exposed employees to potential shock/burn hazards if employee were to contact the electrical connections.  (f) Brown Warehouse Upper Storage Area at SE Wall:  On or about April 8, 2015 and times prior thereto, the District Ranger did not ensure that a junction box had covers over the openings on the top and left sides that were caused by an incorrectly sized cover.   The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 978154, notice number 2, item number 4 and was affirmed as a final order on 12/30/2014, with respect to a workplace located at Bend Pine Administration, Bend Oregon.

(a) Prairie City Compound:  On or about April 8, 2015, the District Ranger did not ensure that fire extinguishers in the warehouse were inspected monthly.  This exposed employees to smoke inhalation hazards from potential delays from having a fire extinguisher not working.
