29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a. On April 7, 2015 at the dock in Fagatogo, the Hyster 60 forklift backrest was removed leaving the mast without a guard and exposing employees to struck-by hazards. b. On April 8, 2015 at the dock in Fagatogo, the Caterpillar 26 forklift seat belt was damaged and could not be used and the horn was not working exposing employees to struck-by hazards.
