29 CFR 1910.179(n)(3)(ii)(c): The hook was not brought over the load in such a manner as to prevent swinging. a) Gantry Area; On or about April 8, 2015, a gantry crane chain and hook was not secured to the d-ring. Inadvertent swinging of the chain and hook caused the hook to hit a stack of hollow structural steel tubing. The stack of hollow structural steel tubing fell and struck an employee resulting in an employee injury.
