29 CFR 1926.502(g)(2)(i): The controlled access zone was not defined by a control line erected not less than 10 feet (3.1 m) nor more than 15 feet (4.5 m) from the working edge.    a) Notre Dame of Bethlehem Elementary School, Worksite, Roof - on or about April 15, 2015, employees were performing masonry work, approximately 12 to 21 feet above grade, where the control lines necessary to prevent employees from approaching the unguarded edges were located less than ten feet from the working edge.

29 CFR 1926.502(g)(3)(iii): Each line used for a Controlled Access Zone did not have a minimum breaking strength of 200 pounds (.88 kN).  a) Notre Dame of Bethlehem Elementary School, Worksite, Roof - on or about April 15, 2015, employees were performing masonry work, approximately 12 to 21 feet above grade, where only a single side of the working area had control lines comprised of non-reinforced plastic caution tape which did not meet the minimum breaking strength of 200 pounds.
