29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) 1120 Zinns Quarry Road, York PA; Plant # 3 - The employer does not ensure that employees utilize procedures developed and documented for the control of hazardous energy when employees setup, adjust, inspect, modify, maintain and/or service machines and equipment. This violation occurred on or about April 13, 2015, where the employer did not require employees to utilize the procedures for equipment and machines such as the Barrel Filler-Spindexer.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) 1120 Zinns Quarry Road, York PA; Plant # 3 - On or about April 13, 2015, the employer had not conducted a periodic inspection of the energy control procedures at least annually. Equipment and machinery within the facility includes but is not limited to the Barrel Filler - Spindexer, Baggers, and Fried Extender. Potential hazardous energies include electrical, pneumatic, and thermal.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition or applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  a) 1120 Zinns Quarry Road, York PA; Plant # 3 - Employees who implement the company's energy control procedures and/or perform servicing or maintenance activities did not receive training in the recognition or applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control on or about April 13, 2015. Employees were exposed to hazards not limited to electrical, thermal, and pneumatic.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) 1120 Zinns Quarry Road, York PA; Plant # 3 - An employee was injured on or about April 13, 2015 when his hand became caught between a conveyor belt drive and machine guard while trying to remove the conveyor belt from a Barrel Filler-Spindexer. The machine was not shut down, locked out, tagged out, and verification of isolation of all energy sources was not performed prior to the work being done.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
