29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h): a/ Ambro Manufacturing, Inc. dba/Special T's, 6 Kings Court Flemington, NJ Site: Screen Printing Area. The lockout program provided by the employer did not include, but not limited to: 1/ the program was not documented to show the limitation of service to be provided by the M & R Operators and the maintenance mechanics. 2/ the program did not have specific steps documented to ensure the equipment was adequately de-energized prior to being serviced or repaired. 3/the program did not have a documented means to control the use and identity of locking devices. 4/the program did not identify designated personnel. Equipment include: M & R Machines and associated electrical equipment. Observed on or about 04/15/2015.
