29 CFR 1926.451(g)(1)(vii): Each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of 29 CFR 1910.451:  On a Pump Jack Scaffold, on or about April 21, 2015, employees were exposed to fall hazards of approximately 15 feet in height to the unpaved ground below when working from a pump jack scaffold without fall protection while performing siding work on a apartment building under construction.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom of the pump jack scaffold system:  On a Pump Jack Scaffold, on or about April 21, 2015, employees were exposed to fall hazards of approximately 15 feet in height to the unpaved ground below when working from a pump jack scaffold that was not secured at the bottom while performing siding work on a apartment building under construction.
