29 CFR 1926.1053(a)(1)(ii): Job-made ladder were not built and tested in conformance with applicable provisions of Appendix A:     a) The cleats/rungs of a job-made ladder provided for employee use were not joined/spliced to the ladder rails as prescribed by American National Standards Institute (ANSI) A14.4-1992 - Safety Requirements for Job-Made Ladders.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      a) Employees were exposed to the hazard of falling approximately 5 feet where a job-made ladder did not extend above the landing surface a foundation excavation.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
