29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   (a) Rock Tenn Co.- Design Studio: An employee was gluing a sample box with an electric glue gun when he felt an electric shock to his left arm. The employee fell on the floor and hit his head. The employee was hospitalized.  The incident occurred on February 3, 2015, but the employer failed to report it to OSHA until February 6, 2015.   Note: Abatement for this violation was previously documented; therefore, the employer is not required to submit abatement certification or documentation for this violation in accordance with 29 CFR 1903.19.
